| 目的 :探讨氧化亚氮 (N2 O)人工流产镇痛的安全性。方法 :回顾性分析自 2 0 0 1年 10月～ 2 0 0 3年 9月的 12 90 0例人工流产 (观察组 :术前 6 0min口服米索前列醇 0 .4mg ,术中采用氧化亚氮吸入镇痛 )的镇痛效果、手术并发症和氧化亚氮不良反应 ,并与 2 0 0 0年 10月～ 2 0 0 1年 9月的 4 16 0例人工流产 (对照组 :术前 6 0min口服米索前列醇 0 .4mg)进行比较。 结果 :观察组的镇痛效果优于对照组 (P <0 .0 1) ;观察组无一例人流综合反应发生 ,与对照组比较 ,差异非常显著 (P <0 .0 1) ,而术中子宫出血、子宫穿孔、宫颈裂伤、漏吸、人工流产不全、术后感染、术后宫腔积血等并发症的发生率在两组间无差异 (P >0 .0 5 ) ;观察组未出现与氧化亚氮有关的严重不良反应。结论 :氧化亚氮吸入应用于人工流产镇痛效果理想、安全 ,不增加手术并发症 ,没有与氧化亚氮有关的严重不良反应发生。 |

| Objective: To evaluate the safety of analgesia-induced abortion by inhaling nitrous oxide. Method: Analgesia-induced abortions were performed in 12 900 early pregnancy women by inhaling nitrous oxide during the operation and taking misoprostol 0.4 mg orally at 60 minutes before the operation (nitrous oxide group) from Oct. 2001 to Sept. 2003. The same operations were performed in 4 160 cases by taking misoprostol 0.4 mg orally at 60 minutes before the operation (control group) from Oct. 2000 to Sept. 2001. The analgesic effect and the operative complications of both groups and side effects concerning nitrous oxide of the nitrous oxide group were observed.Result: The analgesic effect of the nitrous oxide group was better than that of the control group (P< 0.01). There was a significant difference in the syndrome of induced abortion between both groups (0% verse 5.5%, P< 0.01), There was no significant difference in other complications, including perforation of uterus, missed suction, and incomplete abortion between both groups (P> 0.05). There were no severe side effects concerning nitrous oxide in the nitrous oxide group.Conclusion: It's safe to inhale nitrous oxide for analgesia during the abortion. This analgesic method doesn't increase operative complications or have any severe adverse reactions concerning nitrous oxide. |
