| 摘 要 目的：评价实施抗菌药临床应用专项整治活动前后我院妇产科围术期抗菌药应用情况。方法：对我院2011年1～3月和2012年1～3月产科剖宫产手术患者和妇科腹腔镜下Ⅱ类切口手术围术期抗菌药的用药种类、给药时机、用药时限、费用等进行回顾性统计、分析。结果：实施专项整治后，我院产科、妇科一代头孢菌素的使用率明显提高，分别由38.6%和10%提高到98.6%和42%；产科、妇科在术前0.5～2 h（产科在钳夹脐带后）应用抗菌药比例有所提高，分别由44.3%和0提高到95.7%和48%；用药时限＜48 h的比例明显提高，分别由5.7%和0提高到87.1%和42%；平均用药时间则分别由4.97 d和6.0 d下降到1.7 d和2.4 d；平均抗菌药费用分别由1 337元和1 706元下降到403元和747元。结论：抗菌药临床应用专项整治可提高妇产科围术期抗菌药合理应用比例，提升医院合理用药水平。 |

| ABSTRACT Objective:To evaluate perioperative applications of antibiotics for patients in department of gynecology and obstetrics before and after special campaigns of antibiotics.Methods:By retrospective method, the perioperative utilization of antibiotics for patients underwent laparoscopic surgery and uterine-incision delivery during January to March in 2010 and during January to March in 2011 was analyzed statistically in respect of the type of antibiotics, the time selection of administration, the period of drug usage and the costs of antibiotics. Results:After special campaigns of antibiotics, the utilization ratio of first general cephalosporin in department of obstetrics and gynecology of our hospital increased from 38.6% to 98.6% and from 10% to 42% respectively；the proportion of phylactic application of antibacterials 0.5～2 h before operation increased from 44.3% to 95.7% and from 0 to 48% respectively；the proportion of antibacterials after operation within 48 h increased from 5.7% to 87.1% and from 0 to 42% respectively；the average time of antibacterials use was shorten from 4.97d to 1.7d and from 6.0d to 2.4d respectively； the average cost of antibacterials decreased from 1377 yuan to 403 yuan and from 1706 yuan to 747 yuan respectively. Conclusion:Special campaigns of antibiotics can obviously increase the proportion of rational use of antibiotics during perioperative period in Department of Gynecology and Obstetrics and improve the level of rational drug use. |
