| 摘 要 目的：评价综合干预前后我院心外科Ⅰ类切口围手术期预防使用抗菌药现状，提高抗菌药的合理用药水平。方法：采用回顾性方法，对干预前(2012年1～6月)及干预后(2012年7～12月)心外科Ⅰ类切口手术预防用抗菌药的应用种类、给药时机、用药天数等进行统计分析。结果：实施综合干预后，我院心外科围手术期抗菌药预防使用合理率有明显上升，第2代头孢菌素使用率由52.9%提高到81.5%；术前0.5～2 h预防用抗菌药的比例由61.1%提高到99.3%；术后用抗菌药时间≤2 d的比例由2.6%提高到31.5%；治疗使用限制级、特殊级抗菌药微生物送检率达到67.7%、86.9%。结论：实施综合干预可提高我院心外科围术期抗菌药合理用药水平。 |

| ABSTRACT Objective:To evaluate perioperative prophylactic application of antibacterials to type I incision of cardiac operations before and after the comprehensive intervention .Methods:The retrospective method was used to statistically analyze the utilization of antibacterials to type I incision of cardiac operations in our hospital during January to June of 2012 and during July to December of 2012 in respect of the type of antibacterials and time selection of administration.Results:After the comprehensive intervention，the utilization ratio of secondary general cephalosporin in our hospital increased from 52.9％to 81.5％；the proportion of prophylactic application of antibacterials 0.5-2 h before operations increased from 61.1％ to 99.3％；and the proportion of antibacterials after operations within 48 h increased from 2.6％ to31.5％.The level of limitation of drug use and microbial testing rate of special antibacterial treatment reached 67.7% and 86.9%.Conclusion:The comprehensive intervention could obviously improve the level of rational drug use during perioperative prophylactic application of antibacterials. |
