| 摘 要 目的:探讨左炔诺孕酮宫内缓释系统治疗子宫内膜异位症的疗效和安全性，并观察其对患者血清基质金属蛋白酶(MMP)-9及金属蛋白酶组织抑制因子(TIMP)-1水平的影响。方法： 术后复发的子宫内膜异位症患者70例随机分为观察组和对照组各35例。对照组患者月经第1天开始口服米非司酮片25 mg，qd，连用6个月。观察组患者月经来潮后3 d内放置左炔诺孕酮宫内缓释系统。比较两组患者治疗前后血清MMP-9、TIMP-1水平，及MMP-9/TIMP-1比值。观察两组疗效及药品不良反应。结果：治疗6个月后，两组患者的血清MMP-9、TIMP-1水平及MMP-9/TIMP-1比值均有明显下降(P<0.05或P<0.01)，且观察组下降的幅度较对照组更明显(P<0.05)。观察组临床总有效率高于对照组(P<0.05)。两组药品不良反应率比较，差异无统计学意义(P>0.05)。结论：左炔诺孕酮宫内缓释系统治疗子宫内膜异位症疗效较好，安全性好。与其能降低血清MMP-9、TIMP-1水平及MMP-9/TIMP-1比值密切相关。 |

| ABSTRACT Objective::To discuss the curative effect, safety and influence on serum Matrix metalloproteinase-9 (MMP-9) and its Tissue Inhibitor of Metalloproteinase-1 (TIMP-1) levels of Levonorgestrel Releasing Intrauterine System (LNG-IUS) on endometriosis patients. Methods:70 cases of patients, who had been given endometriosis operation and then relapsed, were selected and divided into the observation group and controlled group at random. The patients in the controlled group were given mifepristone tablets 25mg through mouth from the first day of menstrual period once a day for 6 months. The patients in the observation group were given LNG IUS within 3 days upon the menstrual onset. The changes of serum MMP-9 and TIMP-1 levels of patients in the two groups were observed and compared before and after the medical treatment, the changes of MMP-9/TIMP-1 ratio were calculated and the curative effect observation was conducted as well. Results: After 6 months’ medical treatment, the serum MMP-9 and TIMP-1 levels, and MMP-9/TIMP-1 ratio declined more obviously than before (P<0.05 or P<0.01), and the declining rate in the observation group was much higher than that in the controlled group (P<0.05). The total clinical efficiency of patients in the observation group was higher than that in the controlled group (P<0.05). There were no statistical differences in the two groups during the treatment period(P>0.05).Conclusion: LNG IUS had its reliable curative effect on endometriosis with light untoward effect, whose mechanism of action had close effect on reducing the serum MMP-9, TIMP-1 levels and MMP-9/TIMP-1 ratio. |
