| 摘 要 目的:探讨临床药师在重症溃疡性结肠炎患者应用激素治疗中如何开展药学监护工作。方法：临床药师对消化内科1例重症溃疡性结肠炎活动期的典型病例，选择数个具有自身专业特点的立足点，为临床药物治疗提供药学服务。结果：药师以激素治疗方案的选择、药物给药途径及药品不良反应监测等作为切入点，在与医师的合作下，制定个体化给药方案及监护计划，使患者的症状得到控制。结论：临床药师参与临床治疗，应通过药学思维与临床思维相结合，结合药学专业特色，优化临床给药方案，从而利于临床治疗水平的提高。 |

| ABSTRACT Objective::To explore a better pharmaceutical care working process of severe ulcerative colitis patients administrated with glucocorticosteroid for clinical pharmacists.Methods::Individualized pharmaceutical care was provided for one specific severe ulcerative colitis patient administrated with glucocortisteroid by selecting several pharmaceutical problems as entry points. Results::With the consideration of the patient’s condition, clinical pharmacists cooperated smoothly with doctors and nurses and gave their pharmaceutical advices on giving an appropriate dosage, choosing from alternative options of administration methods, taking care of adverse drug reactions, et al. Finally, the patient’s symptom was relieved. Conclusion::In order to optimize the medication therapy and improve the quality of pharmaceutical care, clinical pharmacists should take advantage of their own pharmaceutical specialty and participate in the process of clinical medication. |
