| 摘 要 目的:对二级甲等医院妇产科围手术期患者的抗菌药使用进行药物经济学评估。方法：单纯随机抽取某基层二级甲等医院，选取2010年6月～2011年6月妇产科手术患者300例，提取病历中抗菌药使用的经济学指标，进行成本 效果分析、增量分析、敏感性分析。结果：合理应用抗菌药的Ⅰ类手术切口患者抗菌药成本 效果比为31.49，每增加一个疗效单位抗菌药费用可节省129.23元；合理应用抗菌药的Ⅱ类手术切口患者抗菌药成本 效果比为33.03，每增加一个疗效单位抗菌药可节省73.30元。 结论：药物经济学评估该基层二级甲等医院妇产科围手术期患者抗菌药使用不合理，需加强抗菌药的管理。 |

| ABSTRACT Objective:To study the rationality among the antibacterials by gynecologic and obstetric patients with surgical operation in a second class hospital from the perspective of pharmacoeconomics. Methods:Data of 300 patients undergoing gynecologic or obstetric operation were randomly selected from June 2010 to June 2011 from a second class hospital.The sum of money was investigated and cost effectiveness analysis was used with pharmacoeconomic method. Results: The cost to effectiveness ratio (C/E) of rational use of antibacterials was 31.49.With one unit of antibacterials added, ,the cost would reduce 129.23 Yuan on average in the patient group of incision I. The cost to effectiveness ratio (C/E) of rational use of antibacterials was 33.03, and it could save 73.30 Yuan on average in the patient group of incision I.Conclusion:The application of antibacterials is illegitimate from the perspective of pharmacoeconomics，and the management of antibacterials for gynecologic and obstetric patients with surgical operation should be strengthened in this second class hospital in Shenyang. |
