209例抗感染药物药源性肾损伤不良反应/事件报告分析

      摘 要 目的:了解抗感染药物相关药源性肾损伤的发生情况及特点,为临床用药提供参考。方法:调取2009~2013年解放军不良反应监测中心数据库中抗感染药物相关的药源性肾损伤报告209例,对涉及的患者年龄、性别、药源性肾损伤的药物类别、品种、给药途径、发生时间、实验室指标、临床表现及转归等情况进行描述性统计分析。结果:209例药源性肾损伤中,男149例(71.29%),女60例(28.71%),平均年龄(53.48±23.62)岁。涉及抗感染药品5大类54个品种,引起ADR频次前3位的药物分别为克林霉素、万古霉素、左氧氟沙星。静脉滴注是引起ADR的主要给药途径,占87.08%。ADR的发生时间相对集中,多在用药后1周以内(84.21%)。对可进行肾损伤分期的108例ADR评价,2、3期患者共56例,占51.85%。大部分病例预后良好,其中治愈和好转共185例(88.52%)。结论:抗感染药物相关肾损伤涉及的药物种类和品种较多,且多发生在1周以内,临床应提高对抗感染药物相关肾损伤的认识,并重点关注引发药源性肾损伤频次较高的抗感染药物。

      ABSTRACT Objective:To investigate the conditions and characteristics of drug induced kidney injury of anti infective agents in order to provide the reference for the clinical drug use.Methods: Based on retrospective analysis, 209 drug induced kidney injury of anti infective agents reports were collected from the database of PLA ADR monitoring center between 2009 and 2013. Age and gender of patients, categories of suspected anti infective drugs which induced kidney injury, the variety of drugs, route of administration, occurrence time, laboratory indexes, clinical feature and prognosis were analyzed. Results:Among 209 cases of drug induced kidney injury, there were 149 cases (71.29%) of male patients and 60 cases (28.71%) of female patients, the average age of the patients was (53.48±23.62) years. The ADRs were related to 54 kinds of anti infective agents in 5 categories. The top three drugs were clindamycin, vancomycin, levofloxacin. Intravenous infusion was the main administration route(87.08%). The occurrence time of ADRs was mainly within one week (84.21%). There were 56 patients (51.85%) who are in second and third stage three among 108 cases based on the renal injury evaluation. Most of the cases have good prognosis, 185 cases (88.52%) were cured or improved.Conclusion:The category and variety of the anti infective agents which may induce kidney injury were wide ranging; the ADRs generally occurred within a week. Clinical medical staffs should strengthen the awareness of drug induced kidney injury caused by anti infective agents and pay more attention to the anti infective drugs which induce kidney injury frequently.
