| 摘 要 目的:为临床药师参与心脏移植术后患者药物相互作用的药学监护提供借鉴。方法：临床药师参与1例心脏移植术后患者的治疗过程，并对患者术后药物相互作用进行药学监护，讨论心脏移植术后患者的药物相互作用监护点。结果与结论：患者病情稳定出院。临床药师以药物相互作用的监护为切入，点为心脏移植术后患者提供专业服务，与临床医师、护士形成互补，既保证患者达到免疫抑制的疗效，又减少因药物相互作用导致不良反应的发生，从而保障安全有效的药物治疗。 |

| ABSTRACT Objective:To provide experiences for clinical pharmacist participate in the care of drug interactions of heart transplantation patients.Methods:Clinical pharmacists participated in the treatment process of a heart transplantation patient, analysis on pharmacy practice in the care of drug interactions of the patients after heart transplantation, and discussion monitoring points of drug interactions in heart transplantation patients. Results and Conclusion:The patient was discharged in stable condition. Clinical pharmacists could use the care of drug interactions as a starting point to provide pharmaceutical professional services for heart transplantation patients, which makes an excellent complement to clinicians and nurses, and to ensure safe and effective of drug treatment. Pharmaceutical care make sure the patient achieve the curative effect of immune suppression, and decrease the occurrence of drug adverse reactions induecd by drug interactions, which was the security for safe and effective drug therapy. |
