| 摘 要 目的：总结1例粒细胞缺乏伴发热肿瘤患者和1例误服甲氧氯普胺400mg中毒肿瘤患者的会诊过程。方法：查看患者基本情况，听取医师介绍，查阅病历与文献。结果：病例1为肿瘤热患者，但不能排除感染，药师根据粒细胞减少伴发热的感染谱和本院细菌药敏结果给出排除性治疗方案；病例2采用药物代谢和药理学知识给出会诊意见。患者转归良好。结论：临床药师通过查房积累专业技能，才能在会诊时协助医生拟定治疗方案，提高为患者药学服务的能力。 |

| ABSTRACT Objective:To summarize the clinical consultations with one case of tumor patient with neutropenia and fever, and one case of wrongly used 400 mg metoclopramide poisoning patient. Methods: The basic situation of patient were checked，opinion of doctor were listened and the medical record and clinical literature were looked up. Results:There could not exclude infection because of case 1 was a tumor thermal patients. Diagnostic treatment scheme was made by pharmacist according to bacterial spectrum of neutropenia fever infection and bacterial drug susceptibility results of our hospital. The pharmaceutical consultation was given according to the drug metabolism and pharmacology knowledge for case 2. The patients had a good outcome. Conclusion:Only after clinical pharmacists accumulating the professional skills through daily rounds did we assist physicians developing treatment programs for patients in consultation and improve the ability of pharmaceutical care. |
