      摘 要 目的:探讨术后帕瑞昔布钠持续泵注对髋、膝关节置换术后患者炎症因子和应激反应的抑制作用,为骨科围手术期镇痛提供参考方案。方法:选取52例行全髋、半髋和膝关节表面置换术患者的临床资料进行回顾性分析。按镇痛方法不同分为观察组和对照组,每组26例,对照组术毕予布比卡因自控硬膜外镇痛治疗,观察组术后加用帕瑞昔布钠注射液静脉泵入,5 mg·h-1。观察两组不同时间点疼痛缓解程度,比较两组患者血清炎症因子(IL-6、IL-1β)以及皮质醇 (Cor)、促肾上腺皮质激素(glu)水平变化。结果:观察组术后各时点VAS评分均低于对照组 (P<0.05),且术后各时点疼痛0,I级例数和缓解率均高于对照组(P<0.05)。术后6 h开始,两组血清IL-6、IL-1β水平和Cor和glu水平较术前明显降低 (P<0.05);且观察组上述指标水平均明显低于对照组 (P<0.05)。结论:帕瑞昔布钠持续泵注有利于缓解髋、膝关节置换患者的术后疼痛,降低炎症因子水平和应激反应水平,值得临床推广。

      ABSTRACT Objective: To evaluate the stress reaction and inflammatory factor inhibition effect of parecoxib sodium continuous pumped after hip and knee arthroplasty, and provide a reference for the orthopaedic perioperative analgesic solution.Methods:52 cases of total hip, semi hip and knee joint surface replacement patients’ clinical data were retrospectively analyzed, which divided into observation group and control group according to analgesic method. 26 cases in each group, the control group received bupivacaine for patient controlled epidural analgesia therapy after operation, patients in the observation group combined with parecoxib sodium intravenous injection pumped, 5mg per hour. The level of pain relief in different time points were observed. The plasma inflammatory factor, cortisol (Cor), and the level of were campared in the two groups. Results:The VAS score of observation group was lower than that of the control group in every time points (P<0.05). The 0 and I pain levels of IL-6 and IL-1 in the observation group were significantly higher than those in the control group (P>0.05). The levels of IL-6 and IL-1 were significantly different between two groups (Glu), and the levels of Cor and Glu were significantly lower in two groups (P<0.05). The index level of observation group were significantly lower than the control group (P<0.05).Conclusion:Parecoxib sodium continuous pumped could alleviate the pain of hip and knee replacement in patients with postoperative, reduce the levels of inflammatory factors and stress level, it was worth clinical promotion.
