ABSTRACT Objective:To evaluate the influence of a vancomycin loading dose through a systematic review. Methods: PubMed, Embase, the Cochrane Library and three Chinese literature databases (CNKI, CBM, WanFang) were searched. All studies pertaining to the loading of vancomycin were included. The outcomes included the rate of treatment effectiveness for infections, the proportion of patients reaching target serum concentrations, and the incidence of nephrotoxicity. Results:One RCT and two cohort studies were included in the systematic review, including a total number of 199 patients. Only one study showed the improvement of infection treatment [RR=1.27,95%CI(0.97,1.67),P>0.05],two studies reported the proportion of patients reaching target serum concentrations [RR=1.54,95%CI(0.72,3.30),P>0.05], and one study showed the incidence of nephrotoxicity [RR=3.87,95%CI(0.46,32.57),P>0.05].Conclusion:Although it may improve the improvement of infection treatment and the proportion of patients reaching target serum concentrations, the use of loading dose of vancomycin in the first administration does not show clinical advantages. |
