ABSTRACT Objective:Clinical pharmacists participate in clinical drug therapy, carry out pharmaceutical care on patients, and ensure the patients rational administration of antitumor drugs.Methods:Taking a patient with gastric cancer combined with liver metastasis as example, clinical pharmacists took part in clinical practice, made use of evidence based pharmacy method, combined with the actual conditions of the patient, and solved the problems proposed by clinical, in which the drug most possibly induced thrombocytopenia and its mechanisms were analyzed, then the targeted therapy recommendations were set forward.Results: By means of literature retrieve and clinical analysis, it could infer that the drug caused the patient sudden thrombocytopenia maybe oxaliplatin, and its mechanisms was that the antigen dependent on oxaliplatin could take reaction with platelet antibody. Literature retrieval results showed that the sudden thrombocytopenia often appeared in several times by oxaliplatin treatment, and the clinical conditions of the patient were consistent with the literature; on these grounds, clinical pharmacist assist the clinician formulating the regimen for the patient to elevate platelet, and obtain a significant effect. Conclusion:It could get clinical medicine therapy more reasonable, reducing the harm of ADR on patients, promoting the medication compliance and therapy effects of patients, and the overall medicine therapy level could be improved that clinical pharmacists join to the clinical therapy teams, participate in clinical therapy practice, and carry out pharmaceutical care on patients. |
