| 摘 要 目的：小结临床药师在老年肺炎患者抗感染治疗中的药学监护，以提高临床药物治疗效果。方法与结果：临床药师对1例老年肺炎患者抗感染治疗进行药学监护，及时发现药物相互作用和药品不良反应(ADR)，查阅文献为临床医师提供处理建议，总结ADR发生原因，对医师和护士进行用药宣教，规避用药误区。提高了患者用药的有效性和安全性，患者病情好转后出院。结论：临床药师通过专业的药学知识，为患者提供药学服务，促进临床安全合理用药。 |

| ABSTRACT Objective:To explore pharmaceutical care of clinical pharmacists in the anti infective treatment of elderly severe pneumonia so as to improve therapeutic efficacy of medcine. Methods and Results:Clinical pharmacists provided pharmaceutical services for an elderly pneumonia patient with anti infective treatment, identify drug interactions and adverse reactions (ADR) timely, gave recommendations for clinicians, summarize the causes of adverse reactions by views of literatures, and made medication education for physicians and nurses, to avoid medication errors which help improving the patient’s efficacy and safety of drug use, the patient discharged with controlling infection. Conclusion:Clinical pharmacists used pharmaceutical knowledge to provide pharmaceutical care for patients so as to put forward safe and rational use of drug clinically. |
