      摘 要 目的:利用信息化技术建立高警示药品风险提示系统,减少高警示药品用药差错,保障患者用药安全,提高医疗质量。方法:根据我院药品情况建立高警示药品目录,查阅相关资料,用Excel电子表格整理高警示药品使用过程中的相关风险因素。利用计算机信息化技术,在HIS系统中嵌入高警示药品风险防范模块,在医生开立医嘱及药师审核医嘱界面对高警示药品进行风险提示。结果:采用新系统进行高警示药品风险控制后,不合理医嘱由实施前322条下降至131条,不合理医嘱发生率逐月降低,溶媒选择错误、溶媒剂量错误和用法错误的不合理医嘱数在系统实施前后差异有统计学意义(P<0.05),减少了高警示药品的用药差错,提高了医务人员对高警示药品的认识。结论:利用信息技术建立高警示药品安全屏障,可以减少或避免高警示药品的用药差错,控制高警示药品的使用风险,有利于保障患者用药安全,提高医疗质量。
英文摘要:
      ABSTRACT Objective:To establish a warning system of high alert medication risks by using the information technology , reduce the use of high alert medication drug errors, ensure the safety of medication and improve the medical quality. Methods:According to the situation of medications in our hospital, we established high alert medication directory, reviewed reletive data, used the Excel arranged the risk of high alert medication related risk factors in the process of medication use. Using computer information technology, embedded the high alert risk prevention module in HIS system , warned the risk to the high alert medication on the interface of the doctor’s advice and pharmacist review. Results:After using the new system to monitor the high alert medications , unreasonable orders incidence by prior to the implementation of 322 to 131, the unreasonable orders reduced month by month , Solvent selection error, solvent dosage and usage errors of unreasonable differences before and after the number of orders in the system was statistically significant (P<0.05), the possible high alert medication errors reduced and the knowledge of the medical staff on high alert medications imprved. Conclusion:The use of information technology to establish high alert medication safety barrier, could reduce or avoid high alert medication errors, control the risk of using high alert medication,ensure patient’s medication safety and improve medical quality.
