      摘 要 目的:探讨脾多肽辅助治疗迁延性慢性腹泻(PCDD)患儿的疗效,及对患儿T淋巴细胞(简称T细胞)亚群的影响。 方法:60 例PCDD病例随机分为联合治疗组(30例)和常规治疗组(30例),所有患儿均给予常规口服蒙脱石散、益生菌及补液治疗;联合治疗组同时加用脾多肽静滴辅助治疗。两组疗程均为1周。分别对两组患儿治疗前后外周血T细胞亚群进行检测,以同期体检的20例健康儿童做为健康对照,比较两组患儿治疗前后外周血T细胞亚群变化,评价两组疗效。 结果:治疗前,两组患儿CD3+、CD4+及CD4+/CD8+ 等指标均低于健康对照组(P<0.05)。治疗后,联合治疗组CD3+、CD4+及CD4+/CD8+较治疗前及常规治疗组明显升高(P<0.05),且恢复至健康水平。常规治疗组治疗前后CD3+、CD4+及CD4+/CD8+未见明显变化(P>0.05)。联合治疗组疗效明显优于常规治疗组(P<0.05)。 结论:PCDD患儿存在T细胞介导的免疫功能障碍,脾多肽可恢复PCDD患儿T细胞亚群的正常比例,改善患儿的细胞免疫功能。

      ABSTRACT Objective:To investigate efficacy and change T lymphocyte subsets in peripheral blood of lienal polypeptide in the adjuvant treatment of children with persistent and chronic diarrhea (PCDD). Methods:Sixty children with PCDD were randomly divided to combining therapy group(n=30) and conventional therapy group(n=30). The children of two groups had been given conventional treatment, including oral semectite, probiotics and fluid infusion. The children in combining therapy group were treated with conventional treatment with lienal polypeptide, while the conventional therapy group accepted conventional treatment only. Twenty healthy children who had physical examination during the same period were enrolled as the control group. Flow cytometric analysis was employed to study T lymphocyte subsets in peripheral blood of 60 children with PCDD and 20 healthy controls. Results:Before the treatment, the percentage of CD3+, CD4+ and CD4+/CD8+ of PCDD children were all significantly lower than those of the healthy control group (P<0.05). After treatmeat, CD3+, CD4+ and CD4+/CD8+ in the combining therapy group were significantly higher than before, and those in the conventional therapy group(P<0.05), and returned to the healthy level. While, there were no much changes on CD3+, CD4+ and CD4+/CD8+ in the conventinal therapy group before and after treatment(P>0.05). The efficacy of combining therapy group was significantly better than the conventional therapy group(P<0.05). Conclusion:The T lymphocyte subsets mediated cellular immune function was depressed in children with PCDD. Treatment with lienal polypeptide could recover the imbalance of the ration of T lymphocyte subsets and improve the cellular immune function.
