| 摘 要 目的：分析南京地区34家医院2014~2016年医保新增儿科药物的利用情况，探讨其使用现状及发展前景，并预测其医保支付金额。方法：2014~2016年南京地区医保新增儿科药物的用药数据来自江苏省医药情报研究所。药物利用情况以用药金额、用药频度（DDDs）、限定日费用（DDC）和排序比（B/A）表示。结果：2014~2016年南京地区医保新增儿科药物的利用情况呈增长趋势，说明这些药物越来越得到临床的认可，这也是这些品种纳入医保的原因之一。3年来用药金额和DDDs最高的均为营养药物。各医保新增儿科药物品种中，用药金额靠前的有小儿碳酸钙D3颗粒、小儿复方氨基酸19AA I注射液、健儿消食合剂等，DDDs排名靠前的有小儿碳酸钙D3颗粒、小儿对乙酰氨基酚溶液和小儿布洛芬混悬剂。所有品种连续3年DDC基本稳定。DDC值高的小儿复方氨基酸18AA I注射剂和小儿复方氨基酸19AA I注射剂，其B/A值低。纳入“新医保”后，若DDDs不变，预测医保支付金额为3 243.96万元。结论：2014~2016年南京地区医保新增儿科药物利用基本合理，纳入“新医保”后，不会对医保支付造成很大负担。 |

| ABSTRACT Objective:To analyze the application of new selected pediatric drugs of medical insurance in Nanjing city from 2014 to 2016, and to predict its medicare payments. Methods:The consumption of new selected pediatric drugs and total antitumor drugs in 2014 2016 was obtained from Jiangsu medicine information institute. Drug utilization was expressed as consumption amount, DDDs, DDC and B/A value. Results: From 2014 to 2016, drug utilization of new selected pediatric drugs increased year by year. The top in consumption amount and DDDs was nutrient drugs. Calcium carbonate D3 granula, amino acid 19AA I injection and Jianerxiaoshi mistura were high at consumption amount, while calcium carbonate D3 granula, acetaminophen solution and ibuprofen suspensoid were high at DDDs. DDC of most drugs kept stable relatively. The DDC values of amino acid 19AA I injection and amino acid 18AA I injection, while their B/A values were low. If DDDs have not changed, the estimated medicare payment was 32.44 millions. Conclusion:The application of new selected pediatric drugs of medical insurance in Nanjing city was rational in 2014 2016. Their inclusion of “health insurance” will not give big burden on medical payment. |
