ABSTRACT Objective: To analyze the potentially inappropriate medication (PIM) and its related factors of elderly outpatient prescriptions, we made this research to provide reference for rational drug treatments in the elderly. Methods:Based on Chinese criteria, Beers criteria and STOPP criteria, basic condition (age, gender, hospital number, payment method), clinical diagnosis, and treatment medication of outpatients retrospectively extracted were evaluated and analyzed. Results: In 13 175 elderly patients from 47 104 prescription informations, the number of elderly PIM screening cases were: Chinese criteria 2 415 (18.33%), Beers criteria 2 475 (18.79%), and STOPP criteria 406 (3.08%). The ranking frequency of the top elderly PIM drugs was preceded by clopidogrel, isosorbide mononitrate, aspirin, and estazolam etc. The evaluation results between Chinese criteria and Beers criteria were consistent (Kappa=0.453,P<0.05); Aged ≥80 years old, comorbid disease ≥2, drug types ≥5 kinds, etc. were independent risk factors for elderly PIM (P<0.05). Conclusion: The results evaluated by using Chinese criteria for elderly PIM were similar to those evaluated by Beers criteria. It was necessary to refer to STOPP criteria when revising Chinese criteria. During drug treatments, attention should be paid to adverse drug events probably caused by PIM for the elderly who were over 80 years old, with comorbidities, and with multidrug treatments (≥10). |
