ABSTRACT Objective: To systematically evaluate the efficacy and safety of Yanhuning injection combined with azithromycin in the treatment of mycoplasma pneumoniae in children, and provide the scientific evidence for clinical decision. Methods:PubMed, Embase, Cochrane Library (Issue 5, 2018), Clinicaltrial.gov, SinoMed, CNKI, WanFang Data and VIP databases were electronically searched to collect randomized controlled trials (RCTs) of Yanhuning injection combined with azithromycin in the treatment of mycoplasma pneumoniae in children from inception to September 14, 2018. Two reviewers independently screened literatures, extracted data and assessed risk of bias of included studies. Then Meta analysis was performed by RevMan 5.3 software. Results:A total of 35 RCTs with 3 089 participants were included. The results of Meta analysis showed that: Yanhuning injection combined with azithromycin could achieve a better effect than azithromycin alone in respect of total effective rate (RR=1.18, 95%CI 1.15 to 1.21, P<0.000 01). In addition, the effectiveness of Yanhuning injection combined with azithromycin was superior to the azithromycin alone in cough disappeared time (MD=-2.55, 95%CI -2.75 to -2.34, P<0.000 01), lung rales disappeared time (MD=-3.03, 95%CI -3.26 to -2.80, P<0.000 01), temperature recovery time (SMD=-3.79, 95%CI -4.63 to -2.95, P<0.000 01), X ray disappeared time (SMD=-4.92, 95%CI -6.25 to -3.60, P<0.000 01), average healing time (MD=-3.21, 95%CI -3.44 to -2.98, P<0.000 01). Moreover, comparing with azithromycin alone, Yanhuning injection combined with azithromycin was more effective in decreasing the levels of tumor necrosis factor α (SMD=-1.66,95%CI:-2.87 to -0.45, P=0.007) and interleukin 6 (SMD=-1.44, 95%CI -2.57 to -0.32, P=0.01). Conclusion:The current evidence indicates Yanhuning injection combined with azithromycin is better than azithromycin alone in improving total effective rate and clinical symptoms in the treatment of mycoplasma pneumoniae in children. However, safety still needed for further studies. |
