摘 要 目的:探讨崇明区老年高血压患者用药依从性的影响因素及对策,为老年高血压患者的依从性管理提供参考。 方法:采用配额抽样的方法,使用自拟的调查问卷和Morisky服药依从性量表(morisky medication adherence scale 8 items,MMAS 8)对该地区一、二级医院随访的门诊老年高血压患者进行调研,采用Kruskal Wallis H检验和有序多分类Logistic回归模型对相关影响因素进行分析。 结果:共发放问卷346份,回收有效问卷320份,有效回收率为92.49%。320例患者中,依从性高的有124例(38.75%),依从性中等、低的患者各为98例(30.62%);MMAS 8平均得分为(6.18±1.50)分,依从性高、中等、低患者的得分分别为(7.46±0.46),(6.18±1.50),(4.39±1.32)分。单因素分析结果显示,老年高血压患者用药依从性影响因素中,不同居住地、职业、月收入、合并冠心病或糖尿病、是否接受用药教育等均有统计学意义(P<0.05)。多因素Logistic回归分析结果表明,月收入和是否接受用药教育是影响老年高血压患者用药依从性的主要因素(P<0.05)。高依从性患者的血压达标率明显高于中等、低依从性患者;年龄≥80岁患者的血压达标率明显高于<80岁的患者。 结论:崇明区老年高血压患者的用药依从性水平较低,依从性受诸多因素的影响,药学服务、患者的自我管理以及增加农村老年人的经济收入等有助于提高患者的用药依从性。 |

ABSTRACT Objective: To investigate the influencing factors and solution strategies of medication adherence of elderly patients with hypertension in Chongming district, and provide reference for the medication adherence management. Methods:Quota sampling method, using the self designed questionnaire and the Morisky Medication Adherence Scale (MMAS 8) to conduct outpatients with hypertension in the first and second level hospitals in the region, using Kruskal Wallis H test and multivariate Logistic regression analysis to analyze related factors. Results: A total of 346 questionnaires were distributed and 320 valid questionnaires were collected. The effective recovery rate was 92.49%. Of the 320 patients, 124 (38.75%) had high medication adherence, and 98 (30.62%) had low and medium medication adherence. The average score of MMAS 8 was (6.18±1.50), and the scores of the high, medium and low medication adherence patients were (7.46±0.46), (6.18±1.50), (4.39±1.32) respectively. The univariate analysis showed that among the influencing factors of medication adherence in elderly patients with hypertension, place of residence, occupational distribution, income, diabetes, coronary heart disease and whether to receive medication education have statistical significance (P<0.05). The multivariate Logistic regression analysis showed that income and whether to receive medication education were the main factors affecting medication adherence in elderly hypertensive patients (P<0.05). High medication adherence patients had a higher blood pressure compliance rate than the patients with low or medium medication adherence. The blood pressure compliance rate of patients aged ≥80 years was significantly higher than that of patients aged <80 years. Conclusion:The medication adherence level of elderly hypertension patients in Chongming district is low, and the medication adherence is affected by many factors. The medication adherence of patients can be improved by strengthening the medication education, the self management of patients and increase the economic income of the elderly in rural areas. |
